Requests, cases and applications pass through many hands before they resolve. Workflow and case management is the engineering that lets them move — routed, assigned, reviewed, decided — while a single record keeps the rules, the responsibility and the full history intact.

The handoff problem

Work changes hands. Accountability should not blur.

A request rarely resolves where it begins. It is raised in one place, assigned to another, reviewed by a third and decided by a fourth. At each handover the same questions have to stay answerable: who holds this now, on what basis can it move, what was already decided, and where is the supporting record.

When those answers live in message threads, spreadsheets and separate systems, the process still runs — but its memory does not. Responsibility becomes a matter of recollection, and the history cannot be reconstructed with confidence when it is later questioned.

  4. When a case does not fit the path

A single record is opened at intake and keeps one identity throughout. It is assigned to the responsible unit, then reviewed. Every move between states happens only when a governing rule is met: the case is complete, the unit accepts it, the required conditions and authorization are present. When a case does not fit the normal path it escalates to the level authorized to resolve it and then returns to the governed states, with the escalation kept in the history. An authorized decision is recorded with its basis, and the outcome is closed and retained. Each step is appended to the record's history and never overwritten, so the outcome can always be traced back through the steps that produced it.

What the model holds to

  • One record, one identity

    Intake creates a single case that persists. Everything that follows attaches to it, never to whoever happens to be holding it.

  • Responsibility is explicit

    At any moment the record shows the role that holds the case. A handover is a recorded transfer, not an inference from a mailbox.

  • Transitions answer to rules

    A case moves to the next state only when its conditions are satisfied — the right authorization, the required record, the review a step depends on.

  • Exceptions stay inside the system

    When something does not fit the normal path it escalates through a defined route and returns under control. It is never handled off the record.

  • Nothing is overwritten

    Each step is added, not substituted. The earlier state stays legible, so the outcome can be traced back through the steps that produced it.

  • The outcome is accountable

    A resolved case records what was decided, by whom and on what basis, and carries that account into retention and reporting.
